Otitis Media (Middle Ear infections)

Otitis media, also known as middle ear infections, can occur due to bacteria or viral infections. They are painful as there is inflammation or fluid build-up behind the eardrum. If left untreated, otitis media can damage the middle and inner ear.

 

Common colds and other respiratory infections are the usual culprits of otitis media. The symptoms are similar in children and adults. Some common signs and symptoms include:

  • Pain in ears (otalgia)
  • Blocked ear sensation
  • Hearing loss or diminished hearing levels
  • Giddiness, imbalance, and vertigo
  • Discharge from ears (otorrhea)
  • Difficulty sleeping (more common in children)
  • Fever (more common in children)


Otitis media typically clear up without intervention, but they take a long time to resolve and may happen again. Medication is
often the primary treatment method. However, myringotomy, a small clinical procedure, may be necessary to quicken recovery or treat the complications of this infection.

 

Myringotomy is a procedure where a small cut is made in the eardrum to allow infective material to drain out the middle ear more quickly. A grommet tube may be inserted at the same time. This procedure can be performed in the clinic under local anaesthesia.

 

These uncommon but serious complications may follow ear infections that are not treated promptly, which can lead to severe repercussions such as:

 

  • Loss of hearing – Temporary or permanent inability to perceive sound.
  • Speech or language delay in children This is more likely to occur when there is chronic fluid in the middle ear.
  • Meningitis Inflammation or infection of the tissues surrounding the brain and the spinal cord.
  • Mastoiditis – Inflammation of the mastoid bone (located behind the ear).
  • Ruptured eardrum – The eardrum is responsible for sending sound waves into the inner ear. A damaged eardrum could result in the loss of hearing.

Dr Ker Liang, our ENT specialist from Singapore, will inspect the ear and the eardrum and diagnose the ear condition with the use of an otoscope. If necessary, an examination of the nasal cavity may be performed. Hearing tests will also be conducted, such as the Audiogram and Tympanometry.

As the NUS-NUH Otolaryngology Department Undergraduate Medical Director, Dr Ker Liang supervises the training of medical students from YLLSOM, NUS. She is actively involved
in the training of postgraduate junior doctors and residents in the Head and Neck Surgery department. She was conferred with an Undergraduate Teaching Award by the National University Health System in 2016 for her outstanding efforts as an Otolaryngology educator.
